How to Use This Template

  1. Write the senior citizen’s name
    • Fill in the name of the person in the template.
    • Example: Mak Cik Salmah
  2. Identify the three support zones
    • Red Zone: Emergency help needed immediately.
    • Yellow Zone: Daily or weekly support.
    • Green Zone: Social, emotional, and community support.
  3. List the support people or agencies
    • Write the names of people or organizations that can help.
    • Example: family members, neighbours, clinic, hospital, JKM, PAWE, volunteers, or place of worship.
  4. Write the contact numbers
    • Add the phone numbers for each person or agency.
    • Make sure the numbers are correct and easy to read.
  5. Assign clear roles
    • Write what each person or agency should do.
    • Example: call ambulance, check on the senior citizen, send food, bring medicine, or provide emotional support.
  6. Keep the template in an easy-to-see place
    • Place it on the fridge, near the phone, bedroom wall, or living room.
  7. Save important numbers in the phone
    • Save emergency contacts in the senior citizen’s phone.
    • Use simple names like “Son Emergency”, “Neighbour Help”, or “Clinic”.
  8. Review and update regularly
    • Check the template every 3 months.
    • Update phone numbers, addresses, and support persons if there are changes.
  9. Share the plan with family and neighbours
    • Make sure everyone knows their role.
    • Give a copy to family members or trusted neighbours.
  10. Use it during emergencies
  • In an emergency, refer to the Red Zone first.
  • Call the right person or agency quickly.
